Why Texas Is the Hardest State to Buy Coverage In
The highest uninsured rate in the country
16.7% of Texans have no health coverage, about 5.1 million people, per U.S. Census data. Notably, the next worst state sits near 11.8%.
Premiums jumped 35.2% for 2026
Then 2026 pricing landed. Consequently Texas premiums rose 35.2% on average, the biggest jump since 2018, according to KFF figures reported by the Texas Tribune.
Comparing carriers stopped being optional
That combination is why comparing Texas health insurance plans across carriers stopped being optional this year. In short, the gap between the best and worst plan for you is now worth real money.
Top Carriers We Work With
Types of Texas Health Insurance Plans Available
Custom Health Plans makes navigating open enrollment and finding health insurance options straightforward and stress-free. We partner with you, providing ongoing support and guidance to help you find insurance that covers all your medical needs, whether for an individual, a family or a small business.
Individual Health Insurance
Buying your own coverage? Keep the doctors you already see. We compare PPO, private and off-exchange plans from top carriers and show your real annual cost, not just the monthly premium.
Family Health Insurance
One policy covering your spouse and children, with every family doctor and prescription checked against the network before you enroll. Preventive care and maternity are included on every major medical plan.
Short-Term Health Insurance
Bridge a gap between jobs or before Open Enrollment. Texas allows up to 36 months, one of the longest limits in the country, and coverage can start as soon as the next day.
Small Business Health Insurance
Offer benefits that help you attract and keep good people. Texas businesses can qualify with as few as one eligible employee, and we handle enrollment and compliance for you.
Group Health Insurance
Larger employer coverage with the plan design, contribution strategy and renewal negotiation handled for you, so your team gets stronger benefits without your HR time disappearing every year.
Health Savings Accounts
Pair a lower premium with a savings account that goes in pre-tax, grows tax-free and comes out tax-free for medical costs. The balance rolls over and follows you between jobs.
Fixed Benefit Health Insurance
A set dollar amount paid per medical service, so you know the benefit up front with no deductible math. A budget option with access to a nationwide PPO of providers.
Dental and Vision Insurance
Cleanings, exams, glasses and major dental work that adult medical plans usually leave out. Add it to your health plan or buy it on its own, for yourself or the whole family.
Christian Cost Sharing
Health care sharing programs such as Medi-Share, where members share each other’s medical costs and you set a monthly contribution that fits your budget. These are not insurance, and we will say so plainly.

PPO, HMO, EPO or POS: The Four Letters That Decide Your Access
Plan type matters more than most Texans realize, because it is the part of Texas health insurance plans that controls referrals and out-of-network care.
The four plan types explained:
1️⃣ PPO: broadest access, no referrals
No referral needed and out-of-network care is partially covered. Consequently it carries the highest premium, and in Texas it now sits mostly off-exchange.
2️⃣ HMO: lowest premium, tightest network
A primary care physician coordinates your care and refers you to specialists. However, there is no out-of-network coverage except emergencies.
3️⃣ EPO: no referrals, in-network only
No referral required, although nothing outside the network is covered. In short, a middle option on price.
4️⃣ POS: a hybrid of HMO and PPO
Referrals are required, but some out-of-network care is covered. Similarly, any of the four can be built as an HSA-qualified plan, because the letters describe the network and not the tax treatment.
Private and Off-Exchange Coverage
Off-exchange private plans are bought directly from a carrier rather than through a public exchange, and they carry the same major medical protections as on-exchange Texas health insurance plans.
What private coverage gives you:
1️⃣ The same major medical protections
Specifically, pre-existing conditions are covered, and there are no annual or lifetime dollar limits on essential benefits. However, what you give up is any subsidy.
2️⃣ This is where the PPO networks live
What you often gain is a PPO network, which is increasingly hard to find on-exchange in Texas.
3️⃣ Association health plans
These are off-exchange plans for individuals and families that frequently carry the broadest provider networks available in Texas.
What Moves the Price of Texas Health Insurance Plans
In fact, two people in the same county can be quoted very different premiums for the identical plan.
The six levers on your premium:
1️⃣ Age
Rates climb steadily after 30 and accelerate after 50. This is the single biggest lever on your price.
2️⃣ County
Texas is split into rating areas. For instance, the same plan is priced differently in each one.
3️⃣ Tobacco use
A tobacco surcharge can add a large percentage to the premium, applied on top of the age-rated price.
4️⃣ Deductible level
Lower deductible, higher monthly premium, every time. Conversely, a higher deductible buys a cheaper month.
5️⃣ Plan type
A PPO costs more up front than an HMO, and less in denied out-of-network claims.
6️⃣ Household size
Adding a spouse or dependents changes both the premium and the family deductible. Therefore the lowest premium on a list is not the lowest cost for the year.
Metal Tiers on Marketplace Plans, Explained Without the Jargon
Bronze, silver, gold and platinum are Marketplace labels that describe the cost split between you and the plan.
How the tiers compare:
1️⃣ Bronze and silver
For example, bronze carries the lowest premium and the highest deductible, while silver sits in the middle and is chosen most often.
2️⃣ Gold and platinum
Similarly, gold trades a higher premium for a lower deductible, although platinum is rarely offered in Texas.
3️⃣ Metal tiers do NOT apply off-exchange
Worth knowing: metal tiers do not apply to off-exchange individual coverage, including association plans. Instead, those are described by their actual deductible, coinsurance and network.

Why the Provider Network Matters More Than the Premium on Texas Health Insurance Plans
The network decides who you can see
In short, the network decides which doctors you can see, whereas the premium only decides what you pay to find out.
Directories are re-issued every January
Your physicians should be checked against the carrier’s provider directory and your prescriptions against the drug formulary. Importantly, both are re-issued every January, including on a plan you already hold.
We run both checks before you sign
Therefore we run both checks before you sign. In fact, it is the most common reason someone calls us in February upset about a plan they bought in December.
How It Works (3 Simple Steps)
1. Tell Us About You
First, we need your age, county, household, doctors, prescriptions and budget for Texas health insurance plans. One short call or the form on this page.
2. Compare Your Best Options
Then we put Texas health insurance plans from top carriers side by side, check your doctors against each network, and explain the real annual cost of each.
3. Enroll With Help
Finally, we handle the application, confirm your effective date, and stay on as your broker for renewals and claim questions.
Good Fit vs Not a Fit (Save Time)
This is usually a good fit if:
- You buy your own coverage and have no employer plan
- You run a Texas business and need group or small business coverage
- You want a PPO and keep being shown only HMOs
- You have doctors or prescriptions you are not willing to give up
- You need to bridge a gap between jobs or before Open Enrollment
- You want one place to compare every option rather than one carrier
This may NOT be a good fit if:
- You are 65 or older and eligible for Medicare
- You already have affordable employer coverage that fits your doctors
- You are eligible for Medicaid or CHIP
- You want coverage for a procedure scheduled next week
- You are looking only for auto, home or life insurance

Frequently Asked Questions
What is the best health insurance plan in Texas?
There is no single best plan, because the right one is decided by your doctors and your prescriptions, not by a ranking.
The best of the Texas health insurance plans available to you is the one that keeps the physicians you want at the lowest total annual cost, counting premium, deductible and coinsurance together. For example, for ongoing specialist care that is usually a PPO, whereas for a healthy 30-year-old it is usually an HSA-qualified high deductible plan.
We compare top carriers against your actual doctor list instead of handing you a leaderboard.
How much is health insurance per month in TX?
The average benchmark premium for a 40-year-old Texan is $661 a month in 2026, up from $489 in 2025, per KFF figures reported by the Texas Tribune.
However, your own number depends on your age, county, tobacco use, household size and the deductible you pick. For example, a healthy 30-year-old on a bronze or HSA-qualified plan pays well below that average, while a 60-year-old on a gold PPO pays well above it.
Running your age and ZIP against current rates takes us about ten minutes.
What are the most affordable Texas health insurance plans?
The most affordable Texas health insurance plans are rarely the ones with the lowest premium, and that is the mistake that costs Texans the most money.
For example, a bronze plan with a $9,000 deductible is cheap every month, however it is expensive the moment you need care. Conversely, an HSA-qualified plan often beats it on total cost because the tax deduction offsets part of the deductible.
Therefore, compare the total of premium plus expected out-of-pocket cost for the year instead of the monthly figure alone.
Can I buy my own health insurance in Texas?
Yes. Any Texas resident can buy an individual or family major medical plan without going through an employer.
Specifically, you can enroll during Open Enrollment, or any time you have a Qualifying Life Event such as losing employer coverage, moving, marrying, or having a baby. Additionally, you generally have 60 days from the event.
However, if you have missed both windows, then a short-term plan can cover you until the next Open Enrollment.
Do I pay more for using a health insurance broker?
No. Carrier premiums are filed with the Texas Department of Insurance and are the same whether you buy direct from the carrier or through us.
Instead, our compensation comes from the carrier, and therefore there is no broker fee added to your premium.
That means a second opinion on your plan costs you nothing.
Can I still get a PPO plan in Texas?
Yes, though it is harder than it used to be, because several carriers have moved their individual offerings toward HMO networks.
Specifically, PPO options in Texas now sit largely off-exchange, bought directly from a carrier or through a broker. Additionally, association health plans are another route to a broad network.
If you have been told PPOs are gone from the Texas individual market, that is only true of part of the market.
